Request a sick note for work



Important: Self-certification

You do not require a doctors sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or via the Gov.UK website

Sick (fit) notes

If you have been off work for up to 7 days, you do not need to see your GP for a Med3 Sickness Certificate, you may use an SC2 form (self-certification form).

If you are off work for longer than 7 days, your employer may ask for a Doctors Statement.  To request, please complete an administration, PATCHS, ensuring all of the relevant details including dates are noted. 

If you are unable to use the PATCHS online consultation tool to request a repeat sicknote then please contact the surgery via phone. 

If you have been in hospital during your sickness absence, the ward will provide you with a sickness certificate.
