Adult ADHD Self-Referral Information

If you think you may have ADHD and would like to pursue an assessment, you may be able to self-refer directly to an Adult ADHD assessment service without needing a GP appointment.

Please print off and complete the Adult ADHA Self-Referral in full. 

Once you have completed the form, please bring it into the surgery and hand it to reception. Our team will ensure it is processed appropriately and advise you of any next steps.

Please note that completing the questionnaire does not confirm a diagnosis of ADHD. The information provided will be used as part of the referral and assessment process by the relevant specialist service.
